By completing this form I understand that consent is being provided for the Township of Huron-Kinloss to use my photo or video footage of me or my children on the Township of Huron-Kinloss website, social media accounts, or other marketing materials. 

The type of personal information that may be disclosed on the Township of Huron-Kinloss web site or in a publication is a photograph or video footage.

I/we further understand that this consent may be withdrawn by me/us at any time, upon written notice.
